Southern Flex Rehabilitation was created in 2003 by physical therapists Jeff Hebert and Donna Boue.
We are a rehab staffing network of 70+ therapists—physical, occupational and speech—serving seven parishes throughout southeast Louisiana. A local company, SoFlex personally knows its therapists, their skill sets and their experience.
Therapists undergo an annual peer review and HR assessment, background checks and drug screening as well as licensing verification. Because our therapists are SoFlex employees, and not independent contractors, they can primarily focus on providing quality patient care because we’re managing their employment needs.
SoFlex provides an in-house Rehab Director and two staffing coordinators who serve as liaisons between clients and therapists. Our goal is to provide ASTOUNDING customer service for therapists and clients alike!
Jeff Hebert is co-founder of Southern Flex Rehabilitation & Consulting, LLC. He holds a Bachelor of Science degree in Physical Therapy from LSU School of Allied Health in New Orleans. He has over 20 years of professional experience as a physical therapist in the states of Georgia, New York and Louisiana. Jeff has clinical experience in a variety of patient care settings. He completed a two-year orthopedic manual therapy residency program through Ola Grimsby Institute Consortium in 1997. He formed Southern Flex with Donna Boué in 2003.
Jeff is head of operations at Southern Flex and as CEO he oversees strategic planning, daily business operations and focuses on business development. As an associate member of the Home Care Association of Louisiana, Jeff provides regulatory education to the Southern Flex therapy staff. He also provides agency education regarding issues related to therapy provision in the home. He is a member of the APTA including the Geriatric, Orthopedic and Home Health Sections of the organization, LPTA and HCLA. He is a certified OASIS Specialist.
Donna M. Boué is co-founder of Southern Flex Rehabilitation and Consulting, LLC. She holds a Bachelors of Science degree in Physical Therapy from LSU School of Allied Health in New Orleans. She received a Master in Business Administration from Louisiana State University in Baton Rouge.
As CFO, her responsibilities include financial and cost analysis, project development and management, and employee benefit packages, including 401(k) and insurance administration.
Dedicated to making SoFlex a value-based option for home health agencies, Donna’s goal is to provide cost-efficient, quality therapy services.
As a physical therapist, Donna has expertise in acute care, post-acute care, skilled nursing, long-term care, home health and private practice. She also holds a Master’s degree in business, thus combining clinical skills with business development and strategy. Donna is a certified OASIS specialist.
Professional memberships include: APTA, home health section and private practice section, LPTA, and HCLA.
Alyce Cresson Bowers, Director of Rehabilitation Services, began working for SF as a physical therapist in 2006. She brings ten years of home health experience to our organization in both clinical and managerial skills. Alyce works closely with all SF staff as a clinical mentor promoting collaboration among all disciplines for comprehensive patient care. She also serves as the liaison between SoFlex therapist and HHA.
Alyce is committed to indisputable skilled documentation and case management. Working closely with Jeff and Donna, Alyce provides monthly staff meetings to discuss current topics, policies and changes within our industry. Alyce is responsible for all training, education and orientation for new employees as well as annual evaluations for current therapist employed by SoFlex. She has developed relationships with case managers and agency leaders to provide weekly utilization review and rehab case management.
Alyce has therapeutic skills in the following clinical settings: acute care, skilled nursing care, ICU, out-patient neurologic adult care, home health, aquatic therapy, same day surgery, and long term care in nursing homes. She has been program director for staff development and training for several nursing homes. As a certified clinical OASIS specialist, Alyce helps therapists and agencies achieve OASIS accuracy combined with skilled documentation to assure full benefits for our patients.
Our Front Line
Together, Trina Melerine and Chase Neelis are on the “front line” of Southern Flex operations. Serving as a liaison between the field therapists, Agency case managers, DME providers and patients, they are an invaluable resource to the company. In these supportive roles, Trina and Chase filter accurate and timely communication to all parties involved in providing quality patient care.
Trina has over 16 years of professional healthcare staffing and recruitment experience. As Office Manager, she has been an integral member of the SoFlex team since 2009. Currently, she is responsible for the management and oversight of patient referrals, home health staffing, payroll, billing and general office operations.
Chase earned a Bachelor of Science in Marketing from Louisiana State University in 1999. He acquired over 6 years of experience in hospital staffing, recruitment, and corporate education while employed by a national hospital staffing company. Hired in 2009, Chase is currently responsible for home health staffing, management of computer software and computer-based employee orientation.
